If you have successfully completed your Puppy Passport and the curriculum offered by your OS Certified Trainer™ you are eligible to receive your Socialization Success Certificate in 3 easy steps:

  1. Download the Completion Verification Form and fill out your portion.
  2. Get your OS Certified Trainer™ to complete and sign off on their portion.
  3. Mail or fax the completed form to:
